Hello doctors, its aquib a 19 years old guy this side. I basically have been suffering from ocd and depression from nov 2020 at starting it was mild but during the month of april and may it gets very problematic and I also started feeling panic attacks and hallucinations. Thereafter I consulted a psychiatric doctor in kolkata on june 2021. I told him all my symptoms and after consultation he prescribed me (nexito) escitalopram. Initially he started with 5 mg and taken it to 30 mg I am still having the same dose. The medicine however suited me and I got about 60 to 70 percent relief till nov 2021. But from the past 1 month I again started feeling depression some times mild ocd and hallucinations too. So what should I do now? I didn't missed my doses however.

Dear , it is a good news that you are improving with escitalopram 30 mg itself. Most depressions worsen during winter. Yours may improve with the onset of summer. I don't understand the meaning of hallucinations. If it is one of the symptoms with your ocd, either you may need a small dose of antipsychotic or long term mood stabiliser along with escitalopram. It also depends on your family history and long term history of mood swings, especially mild mania. Since you are young, you can add cbt with erp therapy in addition.

The treatment seems to be going along proper lines. You should convey to your psychiatrist about the 60-70% improvement, and the deficits still present. Many times for persistent depressive disorder like yours, add on medications are needed to address them. Good luck.

Get adequate sleep, at least 8 hours a day. No one can function only on 6 hours of sleep a day. If they can (or so they say), then either they are lying to you about how many hours of sleep ?in total? they get per 24 hours or they are hypomanic. Eat healthily. Exercise regularly, at least some sort of cardiovascular exercise 5 days a week. Minimize your stress level. For example, if you are in college, take 12 credits instead of 16 or 20 credits each semester. Then try to sprinkle a couple easy classes in there. Surround yourself with positive people. I would suggest that you consult a psychiatrist with regards to medication and also consult a psychologist and go for psychological counseling) therapy. A combination of therapy and medication will help best. Especially cognitive behaviour therapy (cbt) can help you manage and deal with depression, it's associated thoughts and behaviours. You are too young. But due to certain reasons if you are on antidepressants you have to take some extra care of yourself.
