I am 47 years old women. I have done my health check up. The value of t3 is 155, t4 is 8.2 and tsh is 6.57. I am currently taking thyrox 25. Can you please suggest me that I should have to continue the same or I have to change the dose.

lybrate-user ji, thanks for the query. I have seen the details mentioned. Since you are on treatment for hypothyroidism for quite some time, so must have been aware that in a well controlled hypothyroid patient tsh should be around 3 to 4 mu/l. At that level all the symptoms are in check. Another aspect is that with a bmi (body mass index) of 31.04 kg/ sq meter, (normal < 23 kg/sq meter) you are already in grade 1 obesity. You have an excess of over 19 kg, which does suggest that the control is not good. So there is a need to increase the dosage to at least 37.5 mcg per day, then check thyroid profile again after 6 weeks to see if desired level is achieved or not. Thanks.

Dear lybrate-user, your test done before starting thyrox 75 ug show only subclinical hypothyroidism - that is t3 and t4 are normal and tsh shows only borderline elevation. Your weight of 77 kg is about 20 kg more than your ideal body weight of about 65-67 kg. I feel that you should focus on a strict low carbohydrate high protein diet, exercise for an hour every day and reduce weight by 1 kg per month to your ideal body weight over the next 2-3 years. Check tpo antibodies and if they show a high value, you need to follow up free t4 and tsh values once in 6 months. If you are antibody negative you need only follow up after 1 year. There is no need to take thyrox now Dr. m s seshadri.
