Hello lybrate user, the condition you are talking about is quiet complex. I will do my best to explain it to you. Tilted pelvis and scoliosis is a form of deformity of the spine and pelvis. It has many causes. In one class of causes, the basic problem arises from the spine itself and includes congenital abnormalities of the individual vertebrae forming the the spine, neurological problems or muscular problems where there is a lack of sufficient strength amongst the muscles of the spine to hold it erect, rare cases of tumors. But the most commonly seen patients have no identifiable cause. We call them idiopathic scoliosis which more common than all the other causes put together. The other class of causes is where the problem is situated elsewhere in the body and the scoliosis is secondary to that problem; for example, lower limb length inequality, ie. Descripancy of the lengths of lower limbs. In such cases, when the patient stands up, due to unequal lengths of the limbs the pelvis becomes tilted. To compensate for the tilted pelvis and to bring back center of gravity the spine bends in the opposite direction and gives rise to a scoliosis secondary to a problem in the lower limbs. The condition is therefore best treated by treating the cause. If the cause is untreatable or no cause is found, then we go for a surgical correction of the spinal deformity. You have to understand that it is a highly complex and risky surgery done only by a handful spine surgeons in india. So, it is advisable to go for surgery only in cases where scoliosis is so severe that it is cosmetically concerning or leads to other health problems. Minor forms can be treated with splints and braces along with back muscle strengthening exercises. If you additionally have breathing problems because of scoliosis you can also do some respiratory (lung) exercises. Surgery would be advisable only in cases where there is a severe scoliosis, other health problems because of scoliosis or a progressively increasing deformity of the spine which mainly happens happens in children. Once the growth of the body is complete, your deformity may progress if already severe or may remain the same through out your life.
