Treatment often works well to reduce the symptoms and distress of OCD greatly. This is called OCD (Obsessive Compulsive Disorder) with poor insight. The reccurent feeling of contamination is the obsession, and her hand washing is the compulsive behavior. Her shouting, temper tantrums are likely secondary to the disorder. This happens due to serotonin (a neurotransmitter) changes in the mind (brain). She requires surely medicines, SSRI's for recovery. But since she has poor insight and atitude towards medicines, the prognosis appears not so good. First get her educated about the illness, and then start treatment.
