I am 23 year old female and I was diagnosed with ocd, generalised anxiety and panic disorder over 7 years ago. I have tried a few different scars over the years, some have really helped and others have not had great side effects so have had to stop. I was recently on lips (fluvoxamine) and have been on 100 mg for 6 weeks. My psychiatrist has decided to take me off of it because it was causing really bad fatigue and gi side effects/rapid weight loss that was interfering with my life. I am now going to try prozac which I think will be good for me. I have read some stories about withdrawing from luvox online and I am very nervous. The plan from my psychiatrist is to drop from 100 mg to 50 mg of luvox for 5 or so days and then stop altogether, then the following day begin prozac at 10 mg for 4 days, then increase to 20 mg and stay on that dose. He said I should not have any issues or any significant issues with withdrawals in this method, as I am going to be replacing the ssri with prozac, but I am still very nervous. Do you think dropping from 100 mg to 50 mg is going to cause me seizures/vomiting or severe withdrawal effects etc? Do you think this way is okay? He said if I am having significant problems I can drop to 25 mg luvox for a few days after 50 mg and then start on the prozac the following day and he was very insistent I would be okay, but I am very worried. Does this change seem okay? I have always cross tapered medicine, never directly switched so I am very scared it's going to cause intolerable withdrawals. Thanks.

Dear Lybrate user, it must be hard going through anxiety. With regard to medications it's best to consult with a psychiatrist. But i'll suggest that along with medications you also go for psychological counseling/therapy. Therapy will help you get to the root of your concern and will help you manage your anxiety and depression with certain strategies and techniques. Especially cognitive behaviour therapy is widely used and proven to treat various anxiety disorders. Once you have learnt to manage anxiety on your own you can also reduce your dependence on medications. Therapy will also help you in the long term. Please consult a psychologist. There is no problem at all in tapering down dose of fluvoxamine. U can go by 25 mg schedule every 4 days and by 12 days you can stop fluvoxamine and while you tapper it down you can start prozac as it will prevent anxiety that will be caused due to ssri (fluvoxamine) withdrawal. U can talk to me or your psychiatrist for detailed treatment. No issues in stopping luvox. All the best.

Dear lybrate-user, both fluvoxamine and prozac are ssris. The anxiety and fear of severe withdrawal is a part of your ocd. Please withdraw from 100 to 50 and stop. No need to go for 25 mg - may not be available. No withdrawal will happen, since it is at a lowest dose. Usually fluoxamine will be withdrawn from 300 - 400 mg/day. Please add therapy also for your treatment so that with erp (exposure and response prevention) your fear comes down - not only for withdrawal but also for other obsessions.
