Most of the breasts have mild inequality in size and shape which is normal. Hence we say that they are sisters, not twins. Sometimes underdevelopment of one breast leads to inequality of the size of breast, but it would be functionally normal. If it is a mild difference and you are not worried about it, then using padding on one side would solve the problem for most of the patients. If the smaller breast is of adequate cup size, then some patients prefer to reduce the size of the bigger breast. This is done via a surgery called as a reduction mammoplasty using a wise pattern skin incision. This leaves one with an anchor shaped scar (it is cosmetically good as most of the scar is in lower part and around areola). If you prefer the larger breast then implant for the smaller one is an option. But if the difference is small, then you may need implants for both breasts to match the size.
