I get pain in the right shoulder, infraspinatus muscle to be precise, closer towards spine. I can move by right arm with full range of motion with out any issues. Also I am do physical activity 5 days a week. Foam rolling and IYT exercise keeps the pain away for some days but it comes back. Pain occurs even while just sitting in a chair. What could be the issue and how can I get it fixed.

Do a CBC count to rule out eosinophilia as some time this can cause muscle pain You must be having g trigger points in the back due to fibromyalgia syndrome_ another possibility which needs examination of your back Please consult.
