Hi I am a 21 years old 1st year mbbs student. I am moderately to morbidly obese and I am loosing weight by portion control and by regular treadmill run. Past 2 days I have a dull pain (not intolerable, but some time felt a little too high) in left iliac fossa region. At first I thought due to abdominal pain due to gas etcc.(i have gas trouble sometimes and it might get to chronic some times. Also I might have lactose intolerance, and lately for past few days I had been drinking strong milked tea). But I am worried because this is the first time the pain is radiating to the left inner thigh, and left lateral scrotum. This pains irritates the left testicle. (again its not localised only to left testis. But also to groin and left lower abdomen, and pain in testis is felt separately ). I had tested cremasteric reflex and the result is positive on both side of scrotum. (even though left side is not as fast and intense contraction as on right). I am really afraid of loosing my left testis. My right testis is smaller than left side and left side one is only the size of an average testis. I am confused whether this pain is due to weakened or damaged muscle in groin (as I was running on treadmill twice a day for few days) or due to problem of testis. As I told I am a moderate obese person, as well I have small amount of gynecomastia. These all really concerning me about my fertility in future. I am not even sure that my right testis is that much working compared to the size of left, and now I am more frightened that will my left testis too undergoes dystrophy.

Get an ultrasound. Of left sided inguinoscrotal area and soft tissue of inner thigh. Get urine routine also and usg whole abdomen kub to rule out Stone. You will get the diagnosis
